点字問題冊子（てんじもんだいさっし）貸（か）し出（だ）しサービス

公益財団法人（こうえきざいだんほうじん）日本国際教育支援協会（にほんこくさいきょういくしえんきょうかい）（以下（いか）、「本協会（ほんきょうかい）」といいます。）は、日本語能力試験（にほんごのうりょくしけん）の点字問題冊子（てんじもんだいさっし）と聴解（ちょうかい）CDを無料（むりょう）で貸（か）し出（だ）します。次（つぎ）の説明（せつめい）を読（よ）んでください。

１．貸（か）し出（だ）しを行（おこな）う点字問題冊子（てんじもんだいさっし）の一覧（いちらん）

点字問題冊子一覧（てんじもんだいさっしいちらん）を見（み）てください。
●問題冊子（もんだいさっし）は、（１）から（８１）まであります。貸（か）し出（だ）しを申（もう）し込（こ）むときは、どの番号（ばんごう）の冊子（さっし）を借（か）りたいか教（おし）えてください。
●一度（いちど）に借（か）りることのできる問題冊子（もんだいさっし）は１人（ひとり）３冊（さつ）までです。貸（か）し出（だ）し期間（きかん）は３０日（にち）です。２０１０年以降（ねんいこう）の問題冊子（もんだいさっし）は貸（か）し出（だ）しません。
●聴解（ちょうかい）CDだけの貸（か）し出（だ）しはできません。

２．このサービスを利用（りよう）できる方（かた）

日本国内（にほんこくない）の視覚（しかく）に障害（しょうがい）がある点字利用者（てんじりようしゃ）またはその代理人（だいりにん）（日本国外（にほんこくがい）では利用（りよう）できません。）

３．貸（か）し出（だ）しの方法（ほうほう）

（１）利用登録（りようとうろく）

初（はじ）めての方（かた）は、利用登録（りようとうろく）をしてください。
●郵送（ゆうそう）かFAXでの登録（とうろく）
「利用登録申請書（りようとうろくしんせいしょ）・登録内容変更届（とうろくないようへんこうとどけ）」をダウンロードして必要（ひつよう）なことを記入（きにゅう）してください。
そして、登録（とうろく）する方（かた）の住所（じゅうしょ）と氏名（しめい）が確認（かくにん）できる証明書（しょうめいしょ）のコピーと一緒（いっしょ）に、郵送（ゆうそう）かFAXで送（おく）ってください。
●入力（にゅうりょく）フォームでの登録（とうろく）
こちら（利用登録申請（りようとうろくしんせい）・登録内容変更入力（とうろくないようへんこうにゅうりょく）フォーム）をクリックして、必要事項（ひつようじこう）を入力（にゅうりょく）してください。

番号（ばんごう）を間違（まちが）えないよう注意（ちゅうい）してください。

●利用登録（りようとうろく）を申（もう）し込（こ）んだ方（かた）には、協会（きょうかい）から郵便（ゆうびん）かEメールで、「登録通知書（とうろくつうちしょ）」で「登録番号（とうろくばんごう）」をお知（し）らせします。「登録番号（とうろくばんごう）」をなくさないよう注意（ちゅうい）してください。
●利用登録（りようとうろく）のときに、Eメールアドレスを教（おし）えてくれた方（かた）には、Eメールで「登録番号（とうろくばんごう）」をお知（し）らせします。
●引（ひ）っ越（こ）しをしたなど、登録内容（とうろくないよう）が変（か）わったときは、「利用登録申請書（りようとうろくしんせいしょ）・登録内容変更届（とうろくないようへんこうとどけ）」に必要（ひつよう）なことを記入（きにゅう）し、本協会（ほんきょうかい）に郵送（ゆうびん）かFAXで送（おく）ってください。または、利用登録申請（りようとうろくしんせい）・登録内容変更入力（とうろくないようへんこうにゅうりょく）フォームから変更（へんこう）してください。本協会（ほんきょうかい）から新（あたら）しい「登録通知書（とうろくつうちしょ）」を送（おく）ります。

（２）貸（か）し出（だ）しの申（もう）し込（こ）み

●郵送（ゆうそう）かFAXでの申（もう）し込（こ）み
「貸出申込書（かしだしもうしこみしょ）」をダウンロードして必要（ひつよう）なことを記入（きにゅう）し、郵送（ゆうそう）かFAXで本協会（ほんきょうかい）に送（おく）ってください。初（はじ）めての方（かた）は、利用登録（りようとうろく）の申請（しんせい）と同時（どうじ）でもいいです。
●入力（にゅうりょく）フォームでの申（もう）し込（こ）み
こちら（貸出申込入力（かしだしもうしこみにゅうりょく）フォーム）をクリックして、必要事項（ひつようじこう）を入力（にゅうりょく）してください。

（３）問題冊子（もんだいさっし）の受（う）け取（と）り

本協会（ほんきょうかい）から問題冊子（もんだいさっし）を送（おく）るときに、「返（かえ）す日（ひ）」をお知（し）らせする文書（ぶんしょ）「日本語能力試験点字問題冊子（にほんごのうりょくしけんてんじもんだいさっし）の貸（か）し出（だ）しについて」も一緒（いっしょ）に送（おく）ります。
利用登録（りようとうろく）のときに、Eメールアドレスを教（おし）えてくれた方（かた）には、Eメールでもお知（し）らせします。

問題冊子（もんだいさっし）を返（かえ）すときは、問題冊子（もんだいさっし）と一緒（いっしょ）に入（はい）っている返信用封筒（へんしんようふうとう）に入（い）れて、郵送（ゆうそう）してください。送料（そうりょう）は無料（むりょう）です。

（４）一度（いちど）に借（か）りられる問題冊子（もんだいさっし）の数（かず）

１人（ひとり）３冊（さつ）までです。
希望（きぼう）する問題冊子（もんだいさっし）が貸（か）し出（だ）し中（ちゅう）のときは、電話（でんわ）かEメールでお知（し）らせします。

（５）貸（か）し出（だ）し期間（きかん）

本協会（ほんきょうかい）が問題冊子（もんだいさっし）を発送（はっそう）した次（つぎ）の日（ひ）から３０日間（にちかん）です。
「返（かえ）す日（ひ）」は文書（ぶんしょ）「日本語能力試験点字問題冊子（にほんごのうりょくしけんてんじもんだいさっし）の貸（か）し出（だ）しについて」か、Eメールでお知（し）らせします。

（６）貸（か）し出（だ）し期間（きかん）の延長（えんちょう）

同（おな）じ問題冊子（もんだいさっし）を次（つぎ）に予約（よやく）している方（かた）がいなければ、１回（かい）だけ（３０日間（にちかん））延長（えんちょう）できます。
延長（えんちょう）するときは、貸（か）し出（だ）し期間中（きかんちゅう）に電話（でんわ）かEメールで連絡（れんらく）してください。
新（あたら）しい「返（かえ）す日（ひ）」を、文書（ぶんしょ）「日本語能力試験点字問題冊子（にほんごのうりょくしけんてんじもんだいさっし）の貸（か）し出（だ）しについて」かEメールでお知（し）らせします。

（７）返（かえ）す方法（ほうほう）

問題冊子（もんだいさっし）や聴解（ちょうかい）CDを返信用封筒（へんしんようふうとう）に入（い）れて、「返（かえ）す日（ひ）」までに本協会（ほんきょうかい）の住所（じゅうしょ）に送（おく）ってください。切手（きって）を貼（は）る必要（ひつよう）はありません。
水（みず）にぬれたり、壊（こわ）れたり、汚（よご）れたりしないように送（おく）ってください。

Braille Test Booklet Lending Service

Japan Educational Exchanges and Services (hereinafter referred to as “JEES”) lends Braille test booklets and listening comprehension CDs for free. Please read the following details

1. List of available Braille test booklets

Please see the list of Braille test booklets.

Test booklets range from (1) to (81). When applying to borrow a test booklet, please let us know the number of the booket(s) you would like.
Up to three test booklets can be borrowed by one person at one time. The lending period is 30 days. Test booklets from 2010 onwards are not available.
Lending of only the listening comprehension CD is not available.

2. Who can use this service?

Braille users in Japan who are visually impaired or their representative (this service is not available outside Japan).

3. How to borrow material

(1) User registration

First time users must complete user registration.
●By mail or FAX
Download and fill in the necessary information in the “User registration application/Change of registered details” form (Japanese).
Once you have completed filling in the form, mail or fax the form with a copy of a form of identification that includes the name and address of the person to be registered.
●Via the input form
Click “User registration application/Change of registered details input form” (Japanese) and input the required information.

Persons who applied for user registration will receive in the mail or e-mail a “Registration notification” and “Registration number” from the JEES. Please do not lose the “Registration number”.
If you provided your e-mail address when submitting your user registration, the “Registration number” will be sent to you by e-mail.
If you have moved or if there are any changes to your registration details, please fill in the appropriate information on the “User registration application/Change of registered details” form (Japanese) and either mail or fax the form to JEES, or click “User registration application/Change of registered details input form” and change details. JEES will send you a new “Registration notification”.

(2) Application for borrowing material

●By mail or FAX
Download and fill in the necessary information in the “Loan application” form(Japanese). Mail or fax the form to JEES. First time users can send this form at the same time as their user registration.
●Via the input form
Click “Loan application input form” (Japanese) and input the required informtaion.

(3) Receiving of test booklets

The “About the lending of JLPT Braille test booklets” (Japanese) that includes the due date for the booklets will also be sent when JEES send test booklets.
If you provided your e-mail address when submitting your user registration, you will be notified by e-mail.

When you return test booklets, please place the test booklets in the self-addressed envelope which is send with test booklets and send them back to JEES. Mailing fee is free.

(4) Number of test booklets that can be borrowed at one time

Up to three booklets per person.
If the test booklets that you requested are currently being loaned to someone else, we will notify you by phone or e-mail.

(5) Loan period

The loan period is 30 days starting from the day following the day of shipment from JEES.
You will be notified of the “Due date” on the “About the lending of JLPT Braille test booklets” document or by e-mail.

(6) Extension of loan periods

You may extend the loan period only once (for another 30 days) and only if there are no other requests pending for the same test booklet.
If you would like an extension, please contact JEES by phone or e-mail during the loan period.
You will be notified of the new “Due date” on the “About the lending of JLPT Braille test booklets” document or by e-mail.

(7) Returning material

Please place the test booklets and listening comprehension CDs in a self-addressed envelope and send them back to the address listed below for JEES by the “Due date”. You do not need to put a stamp.
Please take precautions to make sure that the material does not get wet, dirty or break during delivery.
